Intake evaluation (50 minutes) - $525

New patients will begin with a detailed evaluation to understand your medical, psychiatric and social history thoroughly and allow for the development of the most appropriate and individualized treatment plan.

Follow-up evaluations (25 minutes) - $300

Subsequent follow-up appointments will generally be scheduled at regular intervals to be determined by the doctor.

At this time, Appleby Psychiatry does not accept insurance. We are able to provide a superbill to provide to your insurance company at your request.
